TO ENTER, YOU MUST DOWNLOAD AND COMPLETE THE ENTRY FORM AND SUBMIT BY MARCH 25, 2013 AT MIDNIGHT. PLEASE READ THE OFFICIAL RULES BELOW FOR THIS CONTEST. YOUR SUBMISSION OF THE ENTRY FORM TO THE CONTEST PROVIDER AT SUBMIT@INNOV8FORHEALTH.COM INDICATES YOUR ACCEPTANCE OF THESE OFFICIAL RULES AND THE WEB SITE USE TERMS.
VOID WHERE PROHIBITED. NO PURCHASE NECESSARY. All entrants agree to be bound by these Official Rules.
1. ELIGIBILITY
The Innov8 for Health Business Concept Expo competition (“Competition” or “Contest”) is open to all individuals over the age of 18 at the time of entry, internationally, including healthcare providers, patients, IT professionals, students, researchers, teachers and organizations interested in developing innovative ideas to help achieve the Triple Aim of better health, better care, at lower costs. . The Contest will be conducted by Innov8 for Health (“Contest Provider”). The following are not eligible: employees and representatives of Contest Provider and Sponsors who are involved in the administration, development, fulfillment and execution of this Contest, and the immediate family members (spouse, parent, child, sibling, grandparent, and spouse or "step" of each) and those living in their same households of each such employee or other. By participating, you agree to these Official Rules and the decisions of Contest Provider, which are final and binding in all respects. Void outside of the fifty United States and the District of Columbia, and where prohibited, taxed or restricted by law. All federal, state and local laws and regulations apply.
At least one team member must be a legal U.S. Resident or Citizen throughout the duration of the Contest.
2. CONTEST PERIOD
Submissions for the Innov8 for Health Business Concept Expo can be made beginning at 12:00:01 am Eastern Daylight Time ("EDT") on March 7, 2012 through 11:59:59 pm EDT on April 8, 2012 (the “Contest Period”). The finalists from entries received for each of the two tracks described below (i.e., the University Track and the Start-Up Track) will be announced on www.innov8forhealth.com (the “Web Site”) by April 13, 2012. Each finalist also will receive an e-mail notification. The finalists will all be invited to attend the Innov8 for Health Business Concept Expo to be held in Cincinnati, Ohio on April 20, 2012, where they will present their Contest entries to a panel of judges. At the conclusion of the Innov8 for Health Business Concept Expo on April 20, 2012, Contest Provider will announce four (4) winning teams, two (2) from each of the two tracks described below.
3. ENTRY REQUIREMENTS
All entries must meet the following criteria to be eligible for consideration in the Contest.
► Submissions must be made via the entry form on the Website from March 7, 2012 through April 8, 2012.
► Submissions must describe an innovation, product or service or technology that support the Triple Aim of “better care, better health, lower costs.” The innovation or technology should be appropriate for people in the United States and must be lawfully operated in the United States.
► Individuals and teams can make multiple submissions, but each entry form must contain only one complete innovation.
► The proposed innovation needs to have been created in its ENTIRETY by the submitting team or individual, i.e. it must not infringe or violate the rights of any third parties, including, but not limited to the copyrights, patents, trademarks, trade secrets, and right of publicity/privacy. It is the responsibility of the submitting party to verify all rights.
► Entries must be made in the English language.
4. CONTEST ENTRY TERMS
A. TO ENTER: To enter the Contest, log on to www.innov8forhealth.com (“Web Site”) during the Contest Period. Follow on-screen directions to download, properly complete and submit a registration form. All registration forms must be completed 11:59:59 pm EDT, March 25, 2013 to be valid. For your registration to be valid, you must complete all required information. Your submission of the form to Contest Provider at www.innov8forhealth.com indicates your acceptance of the Web Site use terms and these Official Rules. All entrants must have a valid e-mail address. Information submitted must be in English. The information submitted will be considered the Contest entry ("Entry"). Although you may enter the Contest as many times as you would like (provided each Entry submitted is unique), there is a limit of one (1) prize per person, household and/or email address.
B. CONTENTS OF AN ENTRY: Each Entry must contain a textual submission containing the required information on the Website registration form. We also encourage you to include some type of visual media describing the proposed innovation, such as a diagram, screenshot, doodle, a wireframe, or similar picture or visual aid.
C. CONTEST TRACKS. As part of the registration and entry process, entrants will be required to designate their Entry in one of two innovation tracks: (1) the University Track or (2) the Start-Up Track. The University Track is open to university students enrolled full-time (within the last 3 months) at one of the following universities: the University of Cincinnati; Xavier University; Miami University; Northern Kentucky University; and Mount St Joseph. The Start-Up Track is open to any individual or entity with an interesting, innovative idea that meets the Contest eligibility criteria.
D. SUBMISSION REQUIREMENTS: Entries must be the original work of the entrant; must not infringe any third-party rights; must be suitable for publication (i.e., may not be obscene or indecent); must not contain obscene or pornographic material, may not contain defamatory, slanderous or libelous statements or portray or depict any person, product or entity in a false light, may not invade privacy or other rights of any person, firm or entity, may not in any other way violate applicable laws and regulations, may not contain any reference to any third party trademarks, names or third party logos (other than as owned by the entrant); may not contain any copyrighted works (other than as owned by the entrant); must be truthful and accurate in all respects; may not violate any law, rule or regulation; may not contain any virus, bugs or other deleterious material, and may not contain or depict, for example but not limited to, profanity, defamatory statements, words or symbols widely considered offensive to individuals of any certain race, ethnicity, religious, sexual orientation or socioeconomic group, threats to any person, place, business, group or world peace. Entries also must have a connection to the Greater Cincinnati area and clearly describe a health care innovation. Greater Cincinnati includes Brown, Butler, Clermont, Hamilton and Warren Counties in Ohio; Boone, Bracken, Gallatin, Grant, Campbell, Kenton, and Pendleton Counties in Kentucky; and Dearborn, Franklin, and Ohio Counties in Indiana. A connection to Greater Cincinnati includes: (a) The innovation was invented in Greater Cincinnati; (b) The innovation is the product of a student, staff or researcher of an educational institution in Greater Cincinnati; (c) The company, entity or individual who owns or conceives of the innovation is located in Greater Cincinnati; and/or (d) The inventor is originally from Greater Cincinnati. If you have an alternate connection to Greater Cincinnati, then feel free to describe it on your registration form, but there is no guarantee that it will be accepted. Each entrant warrants and represents that his/her Entry complies with all of these conditions. All Entries will be reviewed for content.
Entries containing prohibited or inappropriate content as determined by the Contest Provider, in its sole discretion, will be disqualified and removed from the Web Site and will not be eligible to win a prize. By entering, you agree that the Contest Provider will make the final determination as to whether Entries are eligible to take part in this Contest. Submission of an Entry grants the Contest Provider and its judges, Sponsors and agents the unconditional right to publish, use, reproduce, adapt, edit and/or modify such Entry in any way in connection with the Contest, in any and all media, without limitation, and without consideration to the entrant, whether or not such Entry is selected as a winning Entry. By submitting an Entry, you agree that your disclosure is gratuitous and without restriction, and will not place the Contest Provider, Contest Sponsors or the Contest judges under any obligation, and that they are free to disclose the ideas contained therein on a non-confidential basis without any additional compensation to you. You acknowledge that, by acceptance of your submission, the Sponsors do not waive any rights to use similar or related ideas previously known to the Sponsors, or developed by their respective employees or contractors, or obtained from sources other than you. By entering, each Entrant grants permission for the use of his/her Entry, and his/her name, and likeness, to be posted on the Internet (including the Web Site) in connection with the Contest, and the advertising, promotion, and publicity of the Contest, and otherwise, as stated in these Official Rules (as solely determined by Contest Provider); and you warrant and represent that you have all right and authority to grant such permission. Notwithstanding anything to the contrary, however, there is no obligation of the Contest Provider or any other person or entity to post or make any use of your particular Entry, or of any your information.
E. CONDITIONS OF SUBMISSION. Only fully completed registration forms shall be eligible for the Contest. Contest Provider reserves the right to disqualify any Entry that it, in its sole opinion, deems inappropriate. General questions concerning the Contest will not be acknowledged or responded to, but technical questions such as Web Site access or problems, can be sent to questions@innov8forhealth.com. Submissions that do not conform to these Official Rules will be disqualified.
5. SELECTION OF WINNERS
A. INITIAL JUDGING PHASE. All Contest Entries must be received by 11:59:59 pm EDT on March 25, 2013. All eligible Entries will be judged our panel of judges, which is comprised of a variety of domain experts with an interest in health care innovation. As designated by you during registration, all Entries will be assigned to one of two innovation tracks, University Track and Start-Up Track. Judging of eligible Entries in each of the two tracks will scored on the following criteria: (i) Effectiveness of the concept to help achieve the Triple Aim of better health, better care, at lower costs; (ii) Scalability of the concept so that it can meet the needs of a large population of people; (iii) Usability and design; (iv) Creativity and innovation; and (v) connection to the Greater Cincinnati area (vi) Qualified Team (vii) Well thought out business plan including thoughts on the problem, the market, and where your product will fit in the marketplace. The judging criteria shall be applied in the sole discretion of the Contest Provider, Sponsors and the individual judges. By participating in the Contest, each entrant acknowledges and agrees that the evaluations may differ by person and agrees to be bound by and not challenge or contest the final decisions of the individual judges. All selections by our panel of judges will be final.
Subject to verification and compliance with these Official Rules, the entrants with the highest scoring Entries in each of the two tracks will be selected as finalists, on or about April 13, 2012. Twelve (12) finalists from each of the two tracks (i.e., 24 teams in total) will be invited to “pitch” their concepts to judges at the Innov8 for Health Business Concept Expo on April 5, 2013.
Attempts to notify potential finalists will be made using the contact information provided on the entrant’s Registration Form. Contest Provider and Contest Sponsors are not responsible for e-mail or other communication problems of any kind. If, despite reasonable efforts, a finalist does not respond within 24 hours of the first notification attempt (or a shorter time as exigencies may require), such finalist will forfeit his or her finalist status and an alternate finalist may be selected. If any finalist is found to be ineligible, or if he or she has not complied with these Official Rules or declines the applicable prize for any reason prior to award, such finalist will be disqualified and an alternate winner may be selected.
B. FINALIST JUDGING PHASE. Each of the 24 finalists selected after the initial judging phase will be invited to “pitch” their business concepts to judges at the Innov8 for Health Business Concept Expo on April 20, 2012. Selected finalists must be able to attend and present his or her concept at the Business Concept Expo on April 12, 2013 Please note that the Business Concept Expo is a full day event, so finalists should make arrangements to be present at the Expo for the entire day. If a finalist is unable to attend, they will not be considered for final judging.
Each of the 24 finalist teams will pitch to a panel of judges on the morning of April 13 2013, and the four (4) highest scoring teams from each track (i.e., 8 teams in total) will be invited to present their business concepts to a panel of judges and to the audience on the afternoon of April 12, 2013.
Two (2) winners from each of the two tracks (4 winners in total) will be announced at the conclusion of the Business Concept Expo on April 12, 2013.
C. PRIZES. Each of the eight (8) winning teams from the morning session of the Expo (i.e., four winning teams from each of the two tracks) will each receive cash prizes of $500.
Each of the four (4) winning teams from the afternoon session of the Expo (i.e., the two highest scoring teams from each of the two tracks) will each receive another cash prize of $1,500.
In addition, winning teams will also be eligible to receive mentorship from industry leaders to further develop their concepts, and may be invited to enter their concepts for inclusion in the Innov8 for Health Startup Accelerator. The odds of winning is dependent upon the number of entries received and strength of each entry based upon the judgment of the judges.
6. LIMITATION OF LIABILITY/RELEASE
No liability or responsibility is assumed by the Contest Provider, the Contest Sponsor(s), and any of their parents, subsidiaries, affiliates, licensees and assigns, and the respective directors, officers, employees, agents, representatives, partners, volunteers, vendors, and contractors of any of the foregoing (collectively, “Contest Parties”), resulting from any user's participation in or attempt to participate in the Contest or download of any information in connection with participating in the Contest. No responsibility or liability is assumed by the Contest Parties for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the Contest: hardware or software errors; faulty computer, telephone, cable, satellite, network, electronic, wireless or Internet connectivity or other online communication problems; errors or limitations of any Internet service providers, servers, hosts or providers; garbled, jumbled or faulty data transmissions; failure of any e-mail transmissions to be sent to or received; lost, late, delayed or intercepted e-mail transmissions; inaccessibility of the Web Site in whole or in part for any reason; traffic congestions on the Internet or the Web Site; unauthorized human or non-human intervention of the operation of the Contest, including without limitation, unauthorized tampering, hacking, theft, virus, bugs, worms; or destruction of any aspect of the Contest, or loss, miscount, misdirection, inaccessibility or unavailability of an account used in connection with the Contest. Contest Provider is not responsible for any typographical errors in the announcement of prizes or these Official Rules, or any inaccurate or incorrect data contained on the Web Site. Use of the Web Site is at the user’s own risk. Contest Provider and the Contest Parties are not responsible for any personal injury or property damage or losses of any kind which may be sustained to any user's or any other person's computer equipment resulting from participation in the Contest, use of the Web Site or the download of any information from the Web Site. By participating in the Contest, the entrant thereby releases the Contest Parties from any and all claims, damages or liabilities arising from or relating to such entrant’s participation in the Contest. By accepting a prize in the Contest, winners agree that the Contest Parties shall not be liable for any loss or injury resulting from participation in the Contest, acceptance or use of any prize, or any travel related thereto.
7. GOVERNING LAW/JURISDICTION
This Contest is offered only in the United States and is governed by the laws of the State of Ohio. Any claims or disputes relating to this Contest, these Official Rules, the FAQs and/or the General Terms and Conditions must be settled in the federal or state courts located in Hamilton County, Ohio.
8. SPONSORS: Innov8 for Health is a community wide initiative sponsored by GE Aviation, Sanofi-aventis, The Health Foundation of Greater Cincinnati, Taft Stettinius & Hollister LLP, and other organizations.
9. WINNERS LIST
Official winners will be posted on the Web Site.
